Dr. Sam Li completed his Interventional Pain Management Fellowship and Anesthesiology Residency at John H. Stroger (Cook County Hospital) in Chicago, IL. During his training, he has become very adept in both ultrasound-guided and fluoroscopically-guided regional and interventional pain procedures.
Dr. Li uses a multidisciplinary, evidence-based approach to relieve his patient’s pain. He believes that each individual’s condition is unique, and therefore focuses on providing a specifically tailored therapy for each individual.
Dr. Li is an active member of Texas Medical Association, Spine Intervention Society, and American Board of Anesthesiology. During his spare time, Dr. Li enjoys spending time with his family, golfing and road biking.
Dr. Eduardo Ozuna completed his Interventional Pain Management Fellowship and Anesthesiology Residency at Texas Tech University Health Sciences Center in Lubbock, TX. During his training, he has become very adept in both ultrasound-guided and fluoroscopically-guided interventional pain procedures.
Dr. Ozuna is passionate about working with his patients to treat the underlying cause of their pain so that they can regain function and improve their quality of life. Dr. Ozuna believes in approaching his patients’ acute/chronic pain with compassion, and he works to develop treatment options using multi-modal, individualized plans that focus on the root of the problem rather than only treating the symptoms.
Dr. Ozuna is an active member of Texas Medical Association, Texas Pain Society, and American Board of Anesthesiology. During his spare time, Dr. Ozuna enjoys spending time with his family, golfing and running.
Dr. Jeffrey Jeng completed his undergraduate degree at the University of California, Berkeley then attended medical school at the University of Texas Health Science Center at San Antonio. After earning his degree, he went to the University of California, San Francisco to complete a year of training in the Department of General Surgery then on to both residency and fellowship at the University of California, Los Angeles in Anesthesiology and Pain Management.
Dr. Jeng is a specialist in complex perioperative patient care and minimally invasive interventional spine, orthopedic, and pain management. He is passionate about helping his patients manage their pain so they can regain function and return to doing the activities they love. He believes in a multimodal, multidisciplinary approach to each patient’s unique pain presentation and closely works with his patients during the process.
Dr. Jeng is an active member of the Texas Medical Association, Spine Intervention Society, and American Society of Regional Anesthesia and Pain Medicine. Dr Jeng’s hobbies include boxing/MMA, spending time with his family, and traveling.
Dr. Alexander Aboka is a Board-Certified, Fellowship trained Orthopedic Surgeon, who received his medical degree from the University of Pittsburgh School of Medicine in Pittsburgh, Pennsylvania. After earning his medical degree and a degree in Public Health, Dr. Aboka completed his orthopedic surgery residency at the University of New Mexico in Albuquerque, and then immediately went on to complete his Orthopedic Sports Medicine, Arthroscopy and Shoulder and Knee Reconstruction fellowship at the Cincinnati Sports Medicine and Orthopedic Center in Cincinnati, Ohio.
With over 15 years of experience and extensive training, Dr. Aboka specializes in minimally invasive arthroscopic surgery of the shoulder, elbow, knee, hip and ankle joints. He also performs shoulder and knee reconstruction and replacement procedures and treats fractures and other injuries and disorders of the musculoskeletal systems, including those of the joints, cartilage, ligaments, tendons, and nerves.
Dr. Aboka is an active member of the American Academy of Orthopedic Surgeons, the Arthroscopy Association of North America and the Texas Medical Association.
Outside of patient care, Dr. Aboka enjoys spending time with his family, traveling and mountain biking.
Dr. Edward Lee is a board-certified, fellowship trained orthopedic surgeon. Dr. Lee is board certified by the American Board of Orthopedic Surgeons and is also an active member of the Arthoscopy Assocation of North America and the Texas Medical Association.
After his undergraduate degree from Columbia University in New York, Dr. Lee completed his intership and orthopedic surgery residency at The George Washington University Schools of Medicine in Washington, D.C. Following his residency, Dr. Lee went to The Mount Sinai School of Medicine in New York for fellowship training in shoulder arthroscopy and reconstruction. Dr. Lee specializes in minimally invasive arthroscopic procedures in the treatment of acute ligament, tendon and cartilage injuries to the shoulder and joints. He also specializes in orthopedic sports medicine injuries and fracture care of the shoulder, elbow, wrist, hand, hip, knee, ankle and shoulder joint replacement.
In this spare time, Dr.Lee enjoys cars, fitness, and spending time with his wife and children.
Dr. Raj V. Kakarlapudi is a Board-Certified Orthopedic Spine Surgeon, who received his medical degree from Creighton University School of Medicine in Omaha, Nebraska. After earning his medical degree, Dr. Kakarlapudi completed his orthopedic surgery residency at The University of Missouri in Columbia, and then immediately went on to complete his orthopedic spine fellowship at the Indiana Orthopedic Hospital in Indianapolis.
With over 15 years of experience and extensive training, Dr. Kakarlapudi specializes in spine surgery, though he can treat injuries and disorders of the muscular and skeletal systems, including those of the joints, ligaments, tendons, and nerves.
Outside of patient care, Dr. Kakarlapudi likes to spend his time traveling and landscaping. He also stays active by running and playing tennis.

Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Please request all prescriptions and authorizations for renewals between 9:00am and 3:00pm, Monday through Friday, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
Forms provided below are various forms that require completion prior to your visit and/or surgery. Please download the appropriate form(s) as directed by your physician and/or staff by clicking on the name of the form. This will allow you to download this software, providing access to the forms. Please bring the completed form(s) with you to your scheduled visit, as well as a complete list of your medications (including strength and frequency). Also please bring your insurance card and driver’s license. This will help expedite the registration process. Thank you.
As of March 2, 2020 Quantum Pain and Orthopedic has contracted ChartSwap to process requests for the release of medical records (protected health information) and billing records.
All patient health care information at Quantum Pain and Orthopedic is confidential. Therefore, medical records cannot be released to any person or organization without the consent of the patient or the patient’s legally authorized representative (unless authorized by law).
Requests for the release of medical records (protected health information) must be submitted in writing and must contain all the elements required by law. Authorizations must also be dated and signed by the patient or the patient’s legally authorized representative. In the case of a minor, a parent or legal guardian must sign the authorization.
Quantum Pain and Orthopedic has authorized forms that can be downloaded.
A current fee schedule for disclosure of health care information is available on the ChartSwap link.
No fee is assessed when the information is being sent to another health care provider or is for a social security disability claim. Requests for medical record information for social security disability claims must be accompanied by a supporting letter from the Social Security Administration.
Patients and Patient representatives will not incur a fee other than the fee associated with ChartSwap. All others will be invoiced thru ChartSwap.
